Dr. Hideyo Noguchi and University of Ghana Medical School


In November 9, 1987, I visited Korle Bu Teaching Hospital.
University of Ghana Medical Scool, P.O. Box 4236, Accra, Ghana.

I surveyed the consultation rooms, wards, theater(operating room) of the 
Hospital. There were poor lighting in operation room, and damaged windows 
of wards. I saw childs with Malaria(Plasmodium falciparum), PCM(Protein-
Calorie-Malnutrition) and meningitis due to Tuberculosis.

Dr. Emmanuel Qvaye Archampong(Dean of Medical school) said
----------- Every year, around 60~70 in medicine and 6~10 in dentisty were 
graduated Ghana University and several docters went to England for study
abroad ----- but few, 5% came back to Ghana ------- .

It was founded in 1979 by the aid of the Japanese government. When I visited
4 Japanese were there. One was Dr. Nakano from the department of Pediatrics,
Mie University. I handed over the drugs such as antibiotics, anti-cold medicine
, anti-diarrhea for children and ointments.

Nowadays, The institute is managed by Ghana.

※Note : Hideyo Noguchi ( 野口 英世1876 – 1928):
In 1913, at the Rockefeller Institute of Medical Research, he demonstrated the presence
of Treponema pallidum (syphilitic spirochete) in the patient's brain of a progressive paralysis
, proving that the spirochete was the cause of the disease.

Dr. Noguchi's name is remembered in the binomial attached to another spirochete,
Leptospira noguchii. In 1928, Noguchi traveled to Africa to confirm his findings.
The purpose of this field work was to test the hypothesis that yellow fever was caused by
spirochaete bacteria instead of a virus. While working in Accra, he died from yellow fever
on May 21, 1928.

※Later studies showed that Yellow fever is caused by yellow fever virus.
To enter Ghana, Liberia and Guinea, required the certification of yellow fever vaccine.

On November 8, took off Cairo at about noon, via Kano and Lagos, we arrived 
8:00 pm in Accra.  So hot, so humid.

Accra (1987): population 860,000.
---------------- 3,000 ~4,000 Lebanese, 1,500 Indians, 1,000 British, 650 Korean, 450 Americans. 
-----------------154 Japanese (80 Overseas Youth Cooperation Volunteers, 6 embassy staffs,
------------------ 4 Noguchi Institute.  Business persons from Nippon Telegraph and Telephone 
------------------ Public Corporation, Nichimen, Marubeni, Itochu, Sumitomo Electric, Sanyo
------------------ Electric Trading, Wakatsuki Fisheries and business travelers-----.) 

November 11: JOVC (Japan Overseas Cooperation Volunteer) in Accra office.
----------------Health consultations were conducted; electrocardiogram and urinalysis

Inhalation therapy for Bronchial asthma

In the case of Bronchial Athma attack, Bronchodilator(Meptin), expectorant
 (Bisolvon) and/or anti-allergic agent (Intal) are used by inhalation devices.

Note (1): Broncho-dilator also acting to the heart beat. Please use, at least, 

-----------4 ~ 6 hours apart.
-------(2): Do not use a mixture of Bisolvon and Intal.

Nile river from air

Next day, November 8, we went to Accra, Ghana. There was only one direct
flight per one week from Cairo to Accra.

After few minutes to departure of Cairo airport, the plane destinaion to
Kano, Nigeria, the green belt appeared along with Nile river.

The driving manner was so bad in Cairo. Everyone rushed to interrupt 

the other lane. I saw so many old cars, which never pass the car inspection 
test in Japan.

                                       
The shopping street in old town------ narrow street, but very interesting place.
-------------------------------------------- worth to visit.

Circumcision and foreskin balanitis

Foreskin balanitis is a bacterial infection of the tip of Penis, which affect
only boys.  The male who performed circumcision have never been affected.

Cairo, Egypt

In November 1987, Dr. Oda and myself went to africa by the request of the 
Ministry of the Foreign Affairs of Japan. The purpuse of the trip was the 
health consultation of the attaches of Japanese Embassy and the Japanese, 
who live in Africa.  The countries included Ghana, Liberia, Guinea, Morocco, 
Tunisia, those all were required VISA.
-------------   Red colour was private passport. green was official passport.
We brought the portable ECG machine and urinalysis kit and some drugs from Japan.
Also, I purchased Chloroquine ( the anti-malaria drug. At present, Mefloquine is the 
standard drug ) from Belgium, since not available in Japan at that time. We stayed 
one night in Cairo.

Dust mites and Bronchial Asthma in children

1) Dust mite live in every home. Nowadays, Japanese house is highly sealed
and less air-circulation. This environment is ideal for ticks and mite.

Dust mites (Dermatophagoides spp.) itself, the debris of dead mite and feces are

the trigger of allergic reaction in some children. The skin contact of Mite's allergen
induce allergic reaction in skin(=Atopic dernatitis) , and inhalation induce allergic
reaction of bronchi, resulting the narowing of air passage ------ wheezing in
Braonchial Asthma.----------
----------i)  frequently change the room air.
---------ii)  bed sheets, carpets are dried to exposure to sunlight in outdoors.
---------iii) The cheap vacuums cleaner is not good, because cheap filter do not 

-------------trap the debris or feces and go through the room.

The vaccine administration techniques in USA

There are 2 route of vaccine injection techniques in USA. One is intramuscular,
other is subcutaneous routes ➞ see Centers for Disease Control and Prevention.
But in Japan, vaccines including Swine Flu are usually administered subcutaneously. 
A little bit different.  Subcutaneous is fewer pain than that of Intramuscular one.

This year, Spring has come early.
Field horsetail (common horsetail) grow in the field, near my clinic.

In my childhood, we ate the boiled ones with sugar and soy sauce.

Bastogne, Belgium

Hitler had the plan to re-occupy Antwerp through the route of Ardennes.
American forces stopped the Hitler plan ------- The battle of bulge
was occurred from December to January, 1944.

Bastogne is located near the border of Luxembourg. A Memorial of the
battle between American force and German was built there.

A small town, Binche, 60km south of Brussels, Belgium.
Carnaval de Binche( Carnival of the Gilles of Binche ) is held in early spring.

Drug preparations for children

There are several types of powder drugs such as granules, dry syrup for children
and liquid medicines are also available.

Basically, Drug dosages are calculated by body surface area or body weights of
each children.  

Roughly, drug dosages of child -----------

----------------12 year old : 2/3 of adult
-----------------7 year old : 1/2 of adult
-----------------3 year old : 1/3 of adult
-----------------1 year old : 1/4 of adult
